Recommended Immunization and Screening Schedule

Birth

•   State Metabolic Screen #1

•   Hearing

•   Hep B

 

4 Weeks

•   Well child assessment

 

2 Months

•   DTaP, IPV, Hib, Hep B, Prevnar, RotaTeq or Rotarix

 

4 Months

•   DTaP, IPV, Hib, Prevnar, RotaTeq/Rotarix

 

6 Months

•   DTaP, Hib, Hep B, Prevnar, RotaTeq, IPV

 

9 Months

•   Well child assessment

•   Hearing screening

 

12 Months

•   Screening for anemia and lead levels

•   MMR Varivax (Chickenpox), Hep A

 

15 Months

•   DTaP, Prevnar, Hib

 

18 Months

•   Hep A

 

2 - 5 Years

•   Blood pressure, dental referral, hearing, vision, urinalysis, TB test

•   DTaP, IPV, MMR, Varivax

 

6-18 Years

•   Physical exam every other year

•   Hematocrit, cholesterol for at-risk patients

•   Tdap or Boostrix at age 11-12

•   Menactra

•   Gardasil (Human Papillomavirus vaccine) series for girls aged 11 years old and older. This vaccine is given as a series at the initial visit and 2 and 6 months from the first injection)
